MicroTESE (Microdissection Testicular Sperm Extraction) is a technique used to study and collect spermatozoa directly from the testicular tissue, performed under operating microscopic guidance; this technique makes it possible to choose the areas of testicle in which it is most likely to find spermatozoa.

The technique involves an incision in the scrotum to allow access to the testicle. A second incision is then performed in the tunic covering the testicle and a microscope is used to identify the most promising areas for sperm retrieval, before concluding the procedure with collections from these specific areas. The spermatozoa collected in this way can be used for both the assisted fertilisation procedure (ICSI) and cryopreservation.

It is usually performed in cases of male infertility involving a complete absence of spermatozoa in the seminal fluid (azoospermia).

This is the most efficacious technique in cases of obstructive and non-obstructive azoospermia.
